2nd Attendee (same practice): With each full registration, one practice associate from within the same practice setting may attend for the reduced price listed below. NOTE: If you register as a second attendee and are attending with a member you are not required to pay a non-member fee. Please select "member" during the registration process if you are attending with a current member.

Trainee: We want to do all we can to help our colleagues-in-training succeed in Lifestyle Medicine. We are happy to provide the below reduced rates for trainees.

Meals are included with your conference registration:

• Sunday 9/30 - Dinner
• Monday 10/1 - Breakfast, Lunch and Dinner
• Tuesday 10/2 - Breakfast, Lunch
• Wednesday 10/3 - Breakfast, Lunch

Significant others are welcome to join us for evening meals with an additional fee; Sunday night $75.00 per person, Monday night $75.00 per person. Please don't register for these meals unless a significant other is planning to attend with you.
